Student Conduct Reporting Form

This form is designed to provide students, employees, faculty, vendors, visitors, or others with an on-line method to report specific information related to an alleged incident(s) where Mesalands Community College’s Student Code of Conduct was violated. Please note that reports related to Title IX should be made using the Title IX Reporting form, and concerning behavior that may not be a violation of Mesalands Community College student code of conduct should be reported using the CARE Team Reporting form.

You are not required to complete the entire form in order for the report to be submitted. Though anonymous reports are accepted, doing so may limit the College’s ability to investigate and respond to a complaint. Mesalands Community College will use the information provided to begin an investigation, which may include contacting the complainant, respondent, and/or any potential witnesses. The College’s ability to respond is improved based on the quantity and quality of information provided.
