Accessibility Reporting Form

This form is designed to provide students, employees, faculty, vendors, visitors, or others with an on-line method to report specific information or complaints regarding accessibility. For example, issues regarding the accessibility of items on the Mesalands Community College website, Moodle Learning Management System, or course materials.

You are not required to complete the entire form in order for the complaint to be submitted. Though anonymous complaints are permitted, doing so may limit the College’s ability to respond to a complaint. The College’s ability to respond is improved based on the quantity and quality of information provided.
