Transcript Request

Requests for transcripts from Mesalands Community College must include:

1. Student’s Name, Address, and Phone Number

2. Social Security Number

3. Student's Signature

4. Date of Birth

5. Name under which you attended the College, if it has changed

6. Address to which you would like the transcript sent

7. Dates of Attendance

8. Date of Completion

Requests will not be processed without the required fee.  The fees for transcripts are as follows

• $4 for an official transcript
• $12 for a transcript by credit card
• $12 for a transcript to be faxed

Acceptable methods of payment: Cash, Personal Check, or Credit Card (Visa, MasterCard, Discover or American Express).
